Access to Restricted Laboratories Permit Request

Instructions:

The purpose of this form is to collect necessary information and documentation required to gain access to restricted laboratories. Incomplete submissions may result in denied access. Before you start, have the following information readily available:

  1. Name/email of individual requiring access to restricted laboratory
  2. UTA ID# of individual requiring access to restricted laboratory
  3. Name/email of Principal Investigator (PI)
  4. Building and Room # Access is needed.

When filling out Permit Date & Time section:

  • Start Date: enter as needed.
  • Expiration Date: for students, enter the end of the current semester; for employees, enter Start Date+12 months. If training is current by the Expiration Date, access will not be revoked.
  • Start Time: enter any time.
  • Expiration Time: enter any time.

When filling out Location section:

  • Select Building only. Do not use Building abbreviation.

Required Trainings and Documentation (link):

  1. General Training Requirement for all labs: CHEM200 Hazard Communication & Waste Management
  2. General Training Requirement for all labs: Site-Specific Training documentation.
  3. Additional Requirement for BSL-2 lab: BIOL500 Biosafety Level 2 (BSL-2).
  4. Additional Requirement for lab handling human/primate blood, tissues, or other potentially infectious materials (OPIM): BIOL200 Bloodborne Pathogens for Laboratory Research Personnel and Hepatitis B Immunization record OR Waiver.
  5. Additional Requirement for lab with Radioactive Materials: RAD100 (if the lab has radioactive materials but the Requester will not use them) OR in-person Radioactive Material User Training (contact EH&S at ehsafety@uta.edu).
  6. Additional requirements for lab with Radiation Producing Machine: RAD200 Radiation Producing Machine (X-Ray)-Part1 and RAD300 Radiation Producing Machine (X-Ray)-Part2 (VIDEO).
  7. Additional requirement for room with Autoclave: BIOL300 On-Site Biohazardous Waste Management: Autoclaving and in-person Autoclave training (proof of completion, certificate or email-confirmation in PDF format).

Lab Hazard - Training/Documents requirements

BSL-2 - CHEM200; BIOL500

BSL-2 with Bloodborne Pathogens - CHEM200; BIOL200; BIOL500; Hepatitis B Immunization record or Waiver.

Radiation - CHEM200; Radioactive Material User Training (to be completed in-person. Contact EH&S at ehsafety@uta.edu). If the lab has radioactive materials but the Requester will not use them, then RAD100 online.

X-Ray - CHEM200; RAD200 and RAD300

Any combination of the above - Accordingly.

BSL-2 with Autoclave - CHEM200; BIOL500 and BIOL300

Once form is submitted a request will be sent to the PI for review and approval before processing. Please allow two (2) business days after PI review is completed for EH&S review and processing. Upload required files (applicable Training Certificates or proof of completion, Hepatitis B records or Waiver) as an attachment. Once the submission has been reviewed, you will receive an email stating whether the request has been approved or denied. If denied, reason for denial will be provided in the email.
