This UAS Application form must be completed and submitted for review and approval by the UTA UAS Review Board (UASRB) prior to any UAS operations on university property or use by UTA employees in the scope of their employment unless excepted by Section IV (E) of the Use of Unmanned Aircraft Systems Application and Approval Procedure CO-CS-PR-08.

An individual affiliated with UTA must submit this application and associated documentation not less than two (2) weeks in advance of flight operations.

Individuals who are not affiliated with UTA must submit this application and associated documentation not less than three (3) weeks in advance of flight operations.

The requestor will receive a UASRB response within 7 business days of this form and all documentation being submitted. Any omission of information requested in this form or failure to provide relevant documentation may result in a delay of processing.

Prior to submission of this form, the Requestor must review CO-CS-PO-07 Unmanned Aircraft Systems Policy.
